Minimum of 2 years medical/surgical experience Wound Care experience preferredĪbility to communicate verbally in writing Revise Plan of Care as indicated and consistent with Accelecare’s Clinical Pathway In collaboration with the Physician and other AWC employees, revise the plan of care and continuously reassess and evaluate until desired outcome is obtainedĬomplete the 4-week reviews in the Multi-Disciplinary Plan of Care and communicate findings with Physician. Implement wound care and HBO protocols as clinically indicatedĬomplete new patient review form and participate in weekly meetingĪccurately complete the superbill and ensure that all orders and charges are initiated accurately Identify patient care situations that require intervention, implement nursing actions and evaluate responsesĭocument a plan of care, in collaboration with Physician, for all assigned patientsįormulate desired outcomes specific to patient’s problems and AWC protocols Record assessment data in an orderly fashion and communicate pertinent informationĪssess, determine classification, and measure wounds accurately and in accordance with policyĪssess compliance to the Clinical Pathway and communicate to the Physician Initiate a nursing assessment of patient’s condition upon admission Recognizes and reports errors or unusual occurrences, and initiates corrective action when appropriate Maintains an orderly, efficient, and timely flow of patients. Documents assessment, planning, implementation and evaluation in the patient record.Develop and maintain a formulary of supplies for the management of wounds, ostomy and continence disorders that incorporates product evaluations, cost-effectiveness comparisons and outcomes quality.Ongoing communication with patients, nursing colleagues and other members of the health care team regarding patient’s status, rendered care, education and treatment plans.Delivery of nursing practices according to the evidence-based research, nursing policies, and standards of care, to provide and ensure safe and excellent delivery of individualized patient centered care.Actively communicates with members of the healthcare team, patient, and patient family Makes accurate, complete, and timely entries into the patient medical record addressing all steps of the nursing process.Collaborates with patient care team members on patient care plan, incorporating consistent use of hand-off communication and SBAR communication methods.Maintains current nursing knowledge to assure a safe working environment.
